I think one of the other repliers on this thread got it right. Alex and Sam might neither be male, and Alex might be talking about some third party. If we assume this is the start of some story, then it would be weird to use a pronoun for someone not yet introduced. Tell my wife that, please.

Ambiguous. I hate trying to avoid this problem, and I get a little annoyed when people solve it with something like "Alex told Sam that Sam was late." I try not to repeat the same word in a sentence unless it's really necessary, or there's some point to the repetition.
