'Malnutrition fits under famine' is very much a product of modern societies where most of the population isn't a huge class of subsistence farmers. Periods of malnutrition are *normal* for pre-modern peasant farmers, as I noted in my series on peasants.

In my book, "malnutrition" fits under famine. I was thinking of countries like South Korea and Japan, where people are not suffering from lack of food, but having fewer kids.
