In my very limited experience you are usually sweating so much from running around with all that gear that peeing is the last thing on your mind.

Yeah and there are different risk calculations for different mental illnesses, symptoms & levels of severity. But it's fairly common for postpartum women to experience intrusive thoughts about hurting their kids, and the vast majority won't follow through and will be okay with outpatient treatment.

So it becomes a question- how do you help a person who struggles with those thoughts recover? Do you treat them as an actual imminent threat forever, now? Or do you help them build trust in themselves and faith that the thought isn't proof that they're a bad person, it's actions that matter.

This is one of those things - when you're talking about intrusive thoughts, mental health professionals will tell you that the thought sticks because it is horrifying to you and violates your values. The vast majority of people who have those kinds of thoughts never commit a crime.

Full disclosure: I had an aunt who much later was diagnosed with bipolar disorder who had a psychotic break after her fourth pregnancy and was hospitalized. They labeled it postpartum psychosis at the time. So I am definitely a bit influenced by that possibility.

I think I just get queasy at the idea that it could be a first psychotic break, which is why I tend to err towards long-term civil commitment.

Yeah, it feels more "I am a failure at this thing I've chosen to do and cannot live with the shame of that so I see no way out except to commit familicide" that we see more typically in men than the kind of "I must save my children from this world/sin" that you typically see in postpartum psychosis.

I think I probably err on the side of "she should be committed long term to a mental institution" but I don't think it's entirely unreasonable for the judicial system to be prosecuting her here, I can see their argument.

She was clearly struggling, but there's very little evidence here that she was struggling to a point that she did not understand that what she did was wrong. It's a big contrast to something like Andrea Yates who had documented religious delusions.

I think the arguments that Lindsay was suffering from postpartum psychosis are less cut and dry than has been portrayed on social media. I haven't seen any evidence her husband was told she was an imminent threat to her children and that he shouldn't leave them alone with her for an hour or two.

Intrusive thoughts, without a plan, are also common in patients with anxiety, depression, or OCD. Lindsay also seems to have not been diagnosed with any postpartum condition, suggesting doctors believed her symptoms began prior to her pregnancy.

At some point she allegedly spoke to her husband about having thoughts about wanting to harm her children, but this was months prior and its unclear she ever told her mental health team about it.

Lindsay Clancy self-admitted to the hospital and they released her when she requested it, so she could attend her child's birthday party. The assumption is that they did not see her as an imminent threat to herself or her children at that time.

Unfortunately I think a lot of this is going to come down to her inpatient clinical notes and whether she was displaying signs of psychosis or mania during that time.
